Dear
Comrades, In many of the SSAs, Family Pension claims are submitted to BSNL SSA
offices after the demise of the Pensioner (non SAMPANN ) and forwarded to CCA
office. While authorising Pension, it is mentioned in PPO about FAMILY Pension. It might have been
submitted to Concerned Branch of the BANK or Post Office. Due to migration of
bank accounts into SAMPANN. Some of the cases are not settled neither by bank
nor by CCA Office.
Following
PROCEDURE to be adopted for SAMPANN PENSIONERS for claim of the FP.
1.Written
request on plain paper by SPOUSE of Deceased ,addressed to Joint CCA with the
following enclosures.
2.Death
certificate XEROX.
3.In case
of joint account or E or S account, No form14 is required. Single account means
form14 required which is available in banks.
4.First
page of Bank Pass Book mentioning Family pensioners Name as account holder. If
single account exists on the name of
Pensioner, new account has to be opened. In case of joint account, Pensioners name
has to be deleted by Bank showing DEATH CERTIFICATE.
5.Xerox
copy of Aadhar.
6.Xerox
copy of PAN card.
7.Declaration
form specimen attached .
8.In case
of migrated Pensioner, One passport size
photo of Claimant FP.is to enclosed.
9.Mobile
number has to be mentioned in the request Letter.
10.All
documents have to be self attested.
11.No role
with BSNL if FP is authorised in PPO. District
Secretaries are requested to note the above points.
